Need your pro help guys. Identify this one!

Just picked this up from some folks who left it in a rental. I can't see any model identifier on it. Obviously it is Sankyo, but everything else is written in Japanese with the exception of what I am guessing is it's serial number in the lower right hand side. It has a bad power supply/transformer(no big deal there). It would seem from what you can check without power that everything else is in order. It also only has one ball that I found rolling around in the case. Anyway any info or translation would be very helpful.
